Job Summary

Seeking a full-time California Licensed Family Nurse Practitioner, the remote position will manage virtual wellness visits, conduct health assessments, and provide comprehensive care coordination while utilizing modern technology to enhance patient care.

Key responsibilities

• Conduct virtual adult and pediatric wellness visits and health assessments
• Collaborate with in-home team members to evaluate patient needs and coordinate care
• Provide comprehensive care management, including preventive care interventions and medication management

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in California
• Credentialed Medicaid provider for the state of California (Medi-Cal)
• Technologically savvy, comfortable using laptops and mobile devices for patient care
• Ability to pass a national background check and valid clinical license search
