California Licensed Health Plan Care Manager

Job Summary

A company is looking for a California Licensed Health Plan Care Manager.

Key Responsibilities:

• Assess, plan, and coordinate care and services for members with behavioral health conditions
• Conduct health risk assessments and develop individualized care plans
• Oversee care support team and monitor member care plan effectiveness

Required Qualifications:

• Active non-restricted California license in Behavioral Health/RN
• Minimum 5 years' experience in healthcare, behavioral health, or substance abuse setting
• Knowledge of utilization management procedures and community resources
• Experience with care management workflows and DSM-5
• Bachelor's degree in Nursing or Master's degree in Counseling or Social Work
