Job Summary

A company is looking for a Home Care Coordinator.

Key Responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services
• Communicate with participants and coordinate with nursing, therapy, aides, and social services
• Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling

Required Qualifications

• Healthcare/medical licensure or equivalency with at least three (3) years of professional experience
• Bachelor's degree preferred
• Minimum of three (3) years of case management or nursing experience in a clinical or home setting
• Nursing knowledge and training to treat frail, elderly participants preferred
