Job Summary

A company is looking for a Home Care Coordinator.

Key Responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services
• Communicate with participants and collaborate with healthcare staff regarding care updates
• Assist with staffing, scheduling activities, and participate in end-of-life care coordination

Required Qualifications

• Healthcare/medical licensure or equivalency with at least three (3) years of professional experience
• Bachelor's degree preferred
• Minimum of three (3) years of case management or nursing experience with a frail or elderly population
• Nursing knowledge and training to treat complicated clinical conditions preferred
