Job Summary

A company is looking for a Home Care Coordinator.

Key Responsibilities

• Conduct in-home care assessments and coordinate care for participants
• Communicate with participants and healthcare providers regarding care updates and scheduling
• Collaborate with the home care team and assist with staffing and scheduling activities

Required Qualifications

• Healthcare/medical licensure or equivalency with at least three (3) years of professional experience
• Bachelor's degree preferred
• Minimum of three (3) years of case management or nursing experience in a clinical or home setting
• Nursing knowledge and training for treating frail, elderly participants preferred
