California Licensed Home Care Coordinator

Job Summary

To enhance participant care, the part-time California Licensed Home Care Coordinator will conduct in-home assessments, coordinate care services, and collaborate with healthcare teams while working a hybrid schedule that includes both remote and onsite responsibilities.

Key responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services regarding care and referrals
• Communicate with participants and healthcare providers to manage schedules, test results, and updates
• Collaborate with Home Care Services staff to monitor home care delivery and participate in end-of-life care coordination

Required qualifications

• Healthcare/medical licensure or equivalency with a minimum of three years of professional experience
• Bachelor's degree preferred
• At least three years of case management or nursing experience in a clinical or home setting with elderly populations
• Nursing knowledge and training to address complicated clinical conditions in frail, elderly participants preferred
