Job Summary

To support a growing team, the part-time California Licensed Home Care Coordinator will conduct in-home care assessments, coordinate care delivery, and collaborate with interdisciplinary teams, combining remote work with required onsite presence in San Bernardino, CA.

Key responsibilities
  • Coordinate incoming calls related to participants, physicians, and agency services
  • Communicate effectively with participants and healthcare staff regarding care updates and scheduling changes
  • Assist with staffing and scheduling activities while participating in end-of-life care coordination
Required qualifications
  • Healthcare/medical licensure or equivalency with at least three (3) years of professional experience
  • Bachelor's degree preferred
  • Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population
  • Nursing knowledge and training necessary for treating frail, elderly participants and managing complicated clinical conditions preferred
