Job Summary
To support a growing home care team, the part-time California Licensed Home Care Coordinator will conduct in-home care assessments, coordinate care for participants, and oversee personal care services while working in a hybrid environment that includes both remote and onsite responsibilities.
Key responsibilities
- Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders, questions, and referrals
- Communicate with participants and collaborate with nursing, therapy, aides, social services, and physicians to manage schedules and updates
- Assist with staffing and scheduling activities, and participate in end-of-life care coordination and support
Required qualifications
- Healthcare/medical licensure or equivalency with at least three (3) years of professional experience
- Bachelor's degree preferred
- Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population
- Nursing knowledge and training necessary to treat frail, elderly participants and manage complicated clinical conditions preferred
