California Licensed Home Care Coordinator

Job Summary

To support a growing home care team, the full-time California Licensed Home Care Coordinator will conduct in-home care assessments, oversee personal care arrangements, and collaborate with interdisciplinary teams, working in a hybrid environment that includes both remote and onsite responsibilities.

Key responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services, addressing questions and referrals
• Communicate effectively with participants and collaborate with nursing, therapy, and social services regarding care updates and scheduling
• Assist in staffing and scheduling activities while participating in end-of-life care coordination and support

Required qualifications

• Healthcare/medical licensure or equivalent, with at least three years of professional experience
• Bachelor's degree preferred
• Minimum of three years of case management or nursing experience in a clinical or home setting with elderly populations
• Nursing knowledge and training necessary to treat frail, elderly participants and manage complicated clinical conditions preferred
