Job Summary

A company is looking for a Housing Navigator to provide support for patients facing housing insecurity.

Key Responsibilities

• Conduct health-related social needs screenings and provide ongoing follow-up with patients regarding housing needs
• Assist members in securing housing and developing budgets, while offering one-on-one counseling and support
• Facilitate connections to community resources and ensure members attend housing-related appointments

Required Qualifications, Training, and Education

• Bilingual Spanish required
• Experience providing peer support to patients with complex health and social conditions
• Training or experience in community health and social determinants of health
• Understanding of mental illness and addiction, with the ability to engage clients effectively
• Knowledge of community-based healthcare systems and the needs of underserved populations
