Job Summary
Managing a direct patient caseload, the full-time California Licensed Lead Care Manager will oversee clinical performance and compliance of a team of Care Managers while maintaining remote patient monitoring and chronic care management standards.
Key Responsibilities
- Manage a defined remote patient monitoring and chronic care management patient panel, ensuring compliance and productivity standards are met
- Conduct regular coaching sessions with Care Managers, utilizing performance data to enhance team outcomes
- Lead client-facing engagements, representing the organization with clinical authority in performance reviews and care coordination discussions
Required Qualifications
- Bachelor's degree in nursing or a closely related healthcare field
- Active and valid registered nurse (RN) license in good standing
- Minimum of three to five years of clinical experience in care management or chronic disease management
- Prior experience in a lead or supervisory clinical role with accountability for team performance
- Working knowledge of remote patient monitoring and chronic care program documentation requirements
