Job Summary

A company is looking for a Lead Case Manager, Bilingual (Spanish).

Key Responsibilities
  • Serve as the primary point of contact for members and their support teams, providing care management and coordination
  • Assess member needs and develop comprehensive Care Management Plans with input from multidisciplinary teams
  • Advocate for members with healthcare professionals and connect them to necessary social services and supports
Required Qualifications
  • Fluency in Spanish is required
  • Bachelor's degree preferred or 2 years of case management experience
  • Ability to work with diverse populations and in various environments
  • Current California driver's license and acceptable driving record
  • Knowledge of community resources and familiarity with trauma-informed care practices
