California Licensed Lead Case Manager

Job Summary

A company is looking for a Lead Case Manager.

Key Responsibilities

• Serve as the primary point of contact for members and their support teams, providing comprehensive case management
• Assess member needs and develop a comprehensive Care Management Plan with a multidisciplinary care team
• Advocate for members, connect them to necessary services, and monitor treatment adherence

Required Qualifications

• High School Diploma or GED required; Bachelor's degree in a related field preferred
• 2+ years of experience in care management or mental health services, particularly with Medi-Cal members
• Current California driver's license with an acceptable driving record
• Experience working with diverse populations and engaging individuals effectively
• Familiarity with trauma-informed care and harm reduction practices
