Job Summary

A company is looking for a Lead Case Manager, Bilingual (Spanish).

Key Responsibilities
  • Serve as the primary contact for members and their support systems, coordinating care with a multidisciplinary team
  • Assess member needs and develop comprehensive Care Management Plans to address gaps in treatment and services
  • Implement Care Plan services, advocate for members, and connect them to necessary social services and supports


Required Qualifications
  • Fluency in Spanish
  • High School Diploma or GED required; Bachelor's degree preferred in a relevant field
  • 2+ years of experience in care management or mental health services with Medi-Cal members
  • Current California driver's license with an acceptable driving record
  • Comfortable working with diverse populations in various environments
