Job Summary
As a bilingual Lead Case Manager in South Los Angeles, the full-time hybrid position will manage comprehensive case management and support services for individuals with persistent mental illness and/or substance use, ensuring personalized care within an integrated health care setting.
Key responsibilities
- Serves as the primary point of contact for members and their support teams, coordinating care and services
- Assesses member needs and develops comprehensive Care Management Plans with input from multidisciplinary teams
- Connects members to necessary social services and advocates on their behalf with healthcare professionals
Required qualifications
- Fluency in Spanish is required
- High School Diploma or GED is required; Bachelor's degree in a related field is preferred
- Minimum of 2 years of experience in care management or mental health services, particularly with Medi-Cal members
- Current California driver's license and acceptable driving record
- Knowledge of community resources and familiarity with trauma-informed care and harm reduction practices
