Job Summary

As a bilingual Lead Case Manager in South Los Angeles, the full-time hybrid position will manage comprehensive case management and support services for individuals with persistent mental illness and/or substance use, ensuring personalized care within an integrated health care setting.

Key responsibilities
  • Serves as the primary point of contact for members and their support teams, coordinating care and services
  • Assesses member needs and develops comprehensive Care Management Plans with input from multidisciplinary teams
  • Connects members to necessary social services and advocates on their behalf with healthcare professionals
Required qualifications
  • Fluency in Spanish is required
  • High School Diploma or GED is required; Bachelor's degree in a related field is preferred
  • Minimum of 2 years of experience in care management or mental health services, particularly with Medi-Cal members
  • Current California driver's license and acceptable driving record
  • Knowledge of community resources and familiarity with trauma-informed care and harm reduction practices
