California Licensed Lead Case Manager

Job Summary

Providing comprehensive case management and support, the full-time hybrid California Licensed Lead Case Manager will assist individuals with persistent mental illness and/or substance use by developing care plans and connecting them to necessary services in West Los Angeles.

Key responsibilities

• Serves as the primary point of contact for members and their support networks, coordinating care with a multidisciplinary team
• Assesses member needs and develops comprehensive Care Management Plans to address gaps in treatment and services
• Implements care plans, advocates for members, and connects them to social services and supports as needed

Required qualifications

• High School Diploma or GED required; Bachelor's degree in social work, psychology, counseling, nursing, or related field preferred
• 2+ years of experience working with Medi-Cal members in care management or mental health services
• Current California driver's license and acceptable driving record
• Familiarity with trauma-informed care and harm reduction practices
• Ability to engage with diverse populations and work in various environments, including members' homes and clinical settings
