Job Summary

To support a growing Remote Patient Monitoring Department, the full-time California Licensed LVN will manage high volumes of inbound and outbound patient interactions, conduct wellness assessments, and provide timely health interventions while working remotely.

Key Responsibilities

• Manage a combination of inbound and outbound telephonic patient interactions in a high-volume clinical call center environment
• Monitor and respond to Remote Patient Monitoring alerts in real time, escalating concerns as necessary
• Accurately document all patient interactions and collaborate with providers to coordinate effective patient care

Required Qualifications

• Active compact LVN/LPN license required
• Proficient with computers, EMRs, and telehealth tools
• Demonstrated AI fluency, actively using tools such as Claude or ChatGPT in daily work
• Ability to work in a fast-paced, high-volume call center environment
• Comfort with performance-based metrics and accountability to targets
