California Licensed LVN Case Manager

Job Summary

A company is looking for a LVN Case Manager to assess health status and manage post-discharge needs.

Key Responsibilities

• Conduct interviews to gather health history and determine health needs
• Collaborate with the RN Care Manager for successful transitions from hospital to home
• Manage post-discharge needs to reduce hospital readmissions and improve patient engagement

Required Qualifications

• High school diploma or GED equivalent
• Current and unrestricted Vocational Nurse License (LVN) in the state of residence
• Two or more years of related LVN clinical experience in a hospital or primary care setting
• Working knowledge of medical terminology and Electronic Medical Records
• Proficient computer skills in MS Word and Outlook
