Job Summary

A company is looking for an LVN Case Manager.

Key Responsibilities

• Assess member needs across various health domains and develop care plans
• Coordinate healthcare benefits and advocate for clients with healthcare professionals
• Maintain patient health records and complete documentation within established timeframes

Required Qualifications

• Active, unrestricted Licensed Vocational Nurse (LVN) license in California
• 2+ years of experience in care management or related fields
• Knowledge of government and community resources related to social determinants of health
• Willingness to work Monday-Friday with potential for evenings and weekends
• Bilingual (English/Spanish) preferred
