California Licensed LVN Case Manager

Job Summary

To support a growing healthcare team, the remote California Licensed LVN Case Manager will conduct clinical evaluations, facilitate care management plans, and collaborate with members and providers to address healthcare needs while working from home.

Key responsibilities

• Conduct clinical evaluations to determine eligibility for case management based on health and psychosocial factors
• Develop and implement personalized case management plans in collaboration with members and healthcare professionals
• Maintain regular outreach to manage a caseload and document interventions and goal achievements

Required qualifications

• Graduation from an accredited Licensed Vocational Nurse program
• Active compact license in California
• 2+ years of recent clinical experience as an LVN/LPN
• 1+ years of experience in care management, disease management, or discharge planning
• Access to a dedicated workspace with high-speed internet for remote work
