California Licensed Medical Case Worker

Job Summary

A company is looking for a Medical Case Worker II to assess healthcare and social needs of clients and provide case management services.

Key Responsibilities

• Conduct initial needs assessments and interviews with clients and their families
• Develop and coordinate case management plans with community health workers and mental health clinicians
• Facilitate out-of-home placements and connect clients with community resources

Required Qualifications

• Bachelor's degree from an accredited college or university
• Professional casework experience in interviewing, counseling, and assisting clients
• Experience with complex case management and home visits
• Bilingual Spanish preferred but not mandatory
• Successful clearance of LA County Live Scan and Medical Clearance Processes
