Job Summary

Serving as a vital link between members and the healthcare system, the full-time California Licensed Member Advocate will conduct outreach, perform community assessments, and assist members in navigating healthcare services to improve health outcomes while working remotely.

Key responsibilities
  • Conduct outreach to identify care gaps and connect members with appropriate healthcare services
  • Perform home visits and community assessments to evaluate member needs and facilitate care coordination
  • Educate members on preventive care and assist with navigating complex healthcare and social service systems
Required qualifications
  • Bachelor's Degree in Social Work, Public Health, Nursing, or a related field, or equivalent experience
  • 2+ years of experience in community health or healthcare quality
  • Experience working with health plan members and navigating community resources
  • Familiarity with Medicaid/Medicare programs and quality measures (e.g., HEDIS)
  • LCSW or RN licensure preferred
