Job Summary
To support a growing pediatric practice, the full-time California Licensed Nurse Care Coordinator will ensure seamless care delivery for children with chronic conditions by coordinating clinical appointments, providing patient education, and managing medication authorizations in a fully remote environment.
Key Responsibilities
- Coordinate and schedule clinical appointments while managing referrals and follow-ups to reduce gaps in care
- Provide evidence-based education to families regarding chronic condition management and respond to inquiries in a timely manner
- Initiate and manage medication prior authorization requests and maintain accurate medication records
Required Qualifications
- Active California Registered Nurse (RN) license in good standing (Associate's, Bachelor's, or Master's degree in Nursing accepted)
- Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems
- Ability to manage competing priorities and work collaboratively in a remote, virtual environment
- Comfort working with diverse, medically and socially complex pediatric populations
- Reliable internet connection and a dedicated workspace suitable for remote work
