Job Summary
Leading a team of clinical staff, the full-time California Licensed Nurse Care Coordinator will ensure seamless care delivery for children with chronic conditions by coordinating clinical appointments, managing medication authorizations, and providing patient education in a fully remote environment.
Key Responsibilities
- Recruit, hire, train, and supervise nursing staff while conducting performance management and development
- Maintain compliance with healthcare regulations and ensure timely documentation and record-keeping
- Communicate effectively with patients, families, and interdisciplinary team members while coordinating care transitions
Required Qualifications
- Active California Registered Nurse (RN) license in good standing (Associate's, Bachelor's, or Master's degree in Nursing accepted)
- Experience leading a team of clinical staff
- Demonstrated proficiency with technology platforms and electronic health record (EHR) systems
- Ability to manage competing priorities in a remote, virtual environment
- Bilingual in English and Spanish
