Job Summary

A company is looking for a Nurse Care Manager for the Pacific Time Zone.

Key Responsibilities

• Manage overall care management of assigned patient panel, leveraging nursing expertise and value-based care operations
• Establish trusting relationships with patients and families to provide clinical and emotional support throughout their care journey
• Coordinate interdisciplinary care plans and monitor the effectiveness of care management strategies

Required Qualifications

• Associate degree in nursing
• Current RN License is required; Compact License preferred
• Care management experience is required
• Certified Case Manager is preferred
• Intermediate skills with MS Office Suite, including Outlook and Teams
