California Licensed Nurse Care Manager

Job Summary

A company is looking for a Nurse Care Manager.

Key Responsibilities:

• Provide preventive health and disease management education
• Schedule and perform video calls and/or phone calls with patients and their caregivers
• Lead a team of care technicians and develop standard operating procedures for care delivery

Required Qualifications:

• Unrestricted RN license in Compact States and CA
• Preferred case management experience
• Strong clinical background with 5+ years of nursing experience
• Experience and/or certification in specialty areas such as cardiology, renal disease, or diabetes is desirable
• Demonstrated leadership ability and experience leading teams
