California Licensed Nurse Care Manager

Job Summary

To support a growing healthcare practice, the remote California Licensed Nurse Care Manager will review electronic health records to extract relevant clinical and behavioral information, create compliant care plans for patients in a Dual Eligible Special Needs Plan, and maintain productivity standards by completing a minimum of 100 care plans per week.

Key responsibilities:

• Review electronic health records to gather pertinent information for care plan development
• Create accurate, complete, and CMS-compliant care plans based on documentation analysis
• Collaborate with managers and quality reviewers to resolve documentation questions and ensure timely care plan completion

Required qualifications:

• Active unrestricted RN license in the state of California
• Minimum of 3 years of clinical experience in care management or related fields
• Strong understanding of CMS compliance requirements and care planning processes
• Proficiency in electronic health records and care management platforms
• Experience with Medicare Advantage or CMS-regulated documentation is preferred
