Job Summary

A company is looking for a Primary Care Provider, Nurse Practitioner / Physician Assistant.

Key Responsibilities

• Provide comprehensive, community-based care to vulnerable seniors
• Build long-term relationships with patients in a value-based care model
• Conduct home visits and manage a remote on-call schedule

Required Qualifications

• Licensed Nurse Practitioner or Physician Assistant
• At least two years of experience working with the elderly
• Prescriptive authority and DEA registration
• Eligibility to serve Medicare/Medi-Cal participants
