Key Responsibilities

• Maintain task category SLAs and respond to patient clinical inquiries
• Engage in clinical decision-making and manage medication adjustments for chronic conditions
• Provide video visits, follow-up care, and collaborate with the care team for complex cases

Required Qualifications

• 5+ years of experience as a Nurse Practitioner or Physician Assistant
• Experience in remote/virtual medical care and chronic disease management
• Expertise in managing high-risk patient caseloads
• Current licensure in California
• Proficiency in CGM and EKG data analysis
