Job Summary

California Licensed Nurse Practitioner, seeking a full-time remote position to conduct telehealth assessments, manage care plans, and drive interventions for home health patients to reduce hospitalizations.

Key Responsibilities

• Conduct initial and follow-up telehealth assessments and coordinate care plans with providers
• Interpret data insights to prioritize high-risk patients and implement timely interventions
• Complete documentation and manage over 25 tele visits per day efficiently

Required Qualifications

• Certified and licensed Nurse Practitioner in California; ANCC certification in relevant specialties
• Eligible for Medicare participation
• Minimum of 2 years NP experience with complex adult populations in home health or post-acute settings
• Comfortable with telehealth visits and virtual care platforms
• Fluent in English and Spanish
