California Licensed Nurse Practitioner

Job Summary

Providing comprehensive virtual primary care, the full-time California Licensed Nurse Practitioner will manage a patient panel through video visits, delivering assessment, diagnosis, treatment, and education while collaborating with a multidisciplinary team in a remote setting.

Key Responsibilities:

• Deliver high-quality virtual primary care, including assessment, diagnosis, treatment, and patient education
• Manage a panel of patients, addressing acute, chronic, and preventive care needs
• Collaborate with the Care and Case Management team to develop and implement effective care plans

Required Qualifications:

• Minimum of four years of experience in primary care within an ambulatory or outpatient setting
• Active, unrestricted Nurse Practitioner and RN licenses in the state of residence, with the ability to obtain additional licenses
• Current board certification by a national certifying body (ANCC or AANP)
• Experience in independently managing a patient panel as the primary care provider
• Demonstrated ability to provide compassionate care across a broad range of medical conditions for all ages
