California Licensed Nurse Practitioner
Job Summary
To support a growing centralized Pre-Enrollment team, the remote California Licensed Nurse Practitioner will conduct thorough chart reviews for PACE applicants, ensuring eligibility and health and safety considerations are met while collaborating with Site Medical Directors and Enrollment RNs.
Key Responsibilities
- Conduct comprehensive chart reviews for PACE applicants, evaluating medical records and clinical documentation
- Collaborate with Site Medical Directors and Enrollment RNs to resolve documentation gaps and present cases as needed
- Provide clinical expertise for Health & Safety determinations, including conducting virtual assessments when necessary
Required Qualifications
- Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in California
- Minimum 3 years of post-licensure NP clinical experience in geriatrics or related settings
- Demonstrated proficiency in chart review and geriatric assessment tools
- Strong ILOC documentation skills meeting California Title 22 and 42 CFR Part 460 standards
- Proficiency with Epic EHR and experience in interdisciplinary team environments
