California Licensed Nurse Practitioner

Job Summary

To provide exceptional telehealth care, the full-time Contractor Nurse Practitioner will coordinate continuity of care for patients recently discharged from the ER or hospital, conduct annual wellness visits, and collaborate with the Primary Care team, all while working remotely.

Key responsibilities

• Coordinate continuity of care and clinical follow-up for discharged patients
• Counsel patients and families on discharge plans, medications, and warning signs
• Document medical histories, conduct wellness visits, and ensure timely completion of patient records

Required qualifications

• Current California Nurse Practitioner license
• Nurse Practitioner certification from ANCC or AANP
• 2+ years of clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care
• Current DEA licensure / Prescriptive Authority post-hire
• Active CPR/BLS Certification
