Job Summary

Providing exceptional telehealth care, the full-time Contract Nurse Practitioner will conduct annual wellness visits, manage high-risk patients, and collaborate with the Primary Care team in a remote setting.

Key responsibilities

• Conduct annual wellness visits and manage high-risk patients primarily through telehealth, with potential in-home visits
• Document medical histories, symptoms, and medication information thoroughly, ensuring timely completion of patient records
• Administer tests and screenings while providing patient education on preventative care and prescribed treatments

Required qualifications

• Current California Nurse Practitioner license
• Nurse Practitioner certification from either the ANCC or AANP
• 2+ years of clinical experience as a Nurse Practitioner in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care
• Working knowledge of chronic care management and various medication options
• Active CPR/BLS Certification and current DEA licensure post-hire
