Job Summary
Delivering exceptional remote care, the full-time California Licensed Nurse Practitioner will manage comprehensive, patient-centered care for adults with complex medical, emotional, and social needs using telemedicine tools.
Key responsibilities
- Develop and implement individualized care plans in collaboration with patients and interdisciplinary team members
- Provide clinical care, including assessment, treatment, and monitoring of acute and chronic conditions
- Address behavioral health concerns and social determinants of health by connecting patients with community resources
Required qualifications
- Active and unrestricted licensure as a Nurse Practitioner in California
- Minimum of 3 years of clinical experience, preferably in primary care with a focus on complex care
- Proven ability to manage a patient panel and provide longitudinal care
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics preferred
- Proficiency in telemedicine technology, including EHR systems and videoconferencing platforms
