Job Summary

A company is looking for a Remote Nurse Health Specialist with a California RN License.

Key Responsibilities

Provide triaging services for patients calling into the virtual care center
Collaborate with healthcare providers to develop and manage patient care plans
Conduct follow-up calls and virtual visits to monitor patient health and address concerns

Required Qualifications

Minimum of 1 year of experience as a Registered Nurse
Successful completion of an accredited Nursing Program; BSN preferred
Current, unrestricted California RN license required
Must have CPR certification
Experience in gerontology or complex care management preferred
