Job Summary

A company is looking for an Oncology Clinical Pharmacy Specialist.

Key Responsibilities

Provide comprehensive medication management, including reconciliation and dose optimization
Partner with healthcare teams to order individualized care plans and promote medication coordination
Monitor therapy effectiveness and safety, and support quality improvement initiatives

Required Qualifications, Training, and Education

Hold a Doctor of Pharmacy (PharmD)
Must be a licensed pharmacist in California
Completion of a PGY2 Oncology Residency, or PGY1 Ambulatory Care Residency with 3 years of oncology experience, or 3 years of clinical pharmacy experience in oncology with 1 year under a Collaborative Practice Agreement
Strong organizational and technical skills
Empathy and a team-first mindset
