Job Summary

A company is looking for an Oncology Clinical Pharmacy Specialist.

Key Responsibilities
  • Provide comprehensive medication management, including reconciliation and dose optimization
  • Partner with healthcare teams to order individualized care plans and promote medication coordination
  • Monitor therapy effectiveness and safety, and support quality improvement initiatives
Required Qualifications, Training, and Education
  • Hold a Doctor of Pharmacy (PharmD)
  • Must be a licensed pharmacist in California
  • Completion of a PGY2 Oncology Residency, or PGY1 Ambulatory Care Residency with 3 years of oncology experience, or 3 years of clinical pharmacy experience in oncology with 1 year under a Collaborative Practice Agreement
  • Strong organizational and technical skills
  • Empathy and a team-first mindset
