Job Summary
A company is looking for an Oncology Clinical Pharmacy Specialist.
Key Responsibilities
- Provide comprehensive medication management, including reconciliation and dose optimization
- Collaborate with healthcare teams to order individualized care plans under a Collaborative Practice Agreement (CPA)
- Monitor therapy effectiveness and safety, and support quality improvement initiatives
Required Qualifications and Education
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in California
- Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with 3 years of clinical oncology pharmacist experience
- At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement (CPA)
- Strong organizational and technical skills
