Job Summary

A company is looking for an Oncology Clinical Pharmacy Specialist.

Key Responsibilities

Provide comprehensive medication management, including reconciliation and dose optimization
Collaborate with healthcare teams to order individualized care plans under a Collaborative Practice Agreement (CPA)
Monitor therapy effectiveness and safety, and support quality improvement initiatives

Required Qualifications and Education

Hold a Doctor of Pharmacy (PharmD)
Must be a licensed pharmacist in California
Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with 3 years of clinical oncology pharmacist experience
At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement (CPA)
Strong organizational and technical skills
