Job Summary

A company is looking for an Oncology Clinical Pharmacy Specialist.

Key Responsibilities

Provide comprehensive medication management, including reconciliation and dose optimization
Collaborate with healthcare teams to order individualized care plans and ensure medication coordination
Monitor therapy effectiveness and safety, making interventions as necessary

Required Qualifications

Doctor of Pharmacy (PharmD) degree
Licensed pharmacist in California
Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with 3 years of clinical oncology experience
At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement
Strong organizational and technical skills
