Job Summary
To elevate patient care at a nationally recognized Academic Medical Center, the part-time California Licensed Oncology Pharmacy Specialist will provide comprehensive medication management, collaborate with care teams under a Collaborative Practice Agreement, and conduct patient assessments both virtually and in-person.
Key responsibilities
- Provide comprehensive medication management, including reconciliation and dose optimization
- Partner with interdisciplinary teams to order individualized care plans and promote medication coordination
- Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as appropriate
Required qualifications
- Doctor of Pharmacy (PharmD) degree
- Active pharmacist license in California
- Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with at least 3 years of clinical oncology pharmacist experience
- At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement (CPA)
- Strong computer skills, including proficiency in Microsoft Office tools
