Job Summary

A company is looking for a Patient Care Advocate to improve member and provider experiences through outreach and education.

Key Responsibilities
  • Act as a liaison between members, families, and healthcare providers to close care gaps
  • Conduct telephonic outreach and face-to-face education regarding care gaps and preventive services
  • Schedule appointments and arrange transportation and community resources for members as needed
Required Qualifications, Training, and Education
  • Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related field, or equivalent work experience (4 years total required)
  • 2+ years of experience in direct patient care, social work, quality improvement, or health coaching, preferably in a managed care environment
  • Preferred certifications include Licensed Practical Nurse (LPN), Licensed Master Social Work (LMSW), Certified Social Worker (C-SW), Licensed Social Worker (LSW), or Licensed Registered Nurse (RN)
  • Must be authorized to work in the U.S. without employment-based visa sponsorship
  • Preference for bilingual Spanish speakers
