Job Summary

To support a growing interdisciplinary care team, the full-time California Licensed Patient Intake Coordinator will conduct patient screenings, assist with treatment coordination, and ensure seamless patient experiences in a remote setting.

Key Responsibilities
  • Conduct patient screening calls and assist with intake form completion and onboarding processes
  • Collaborate with clinical teams to support treatment planning, appointment scheduling, and care coordination
  • Document patient interactions and maintain compliance with clinical, legal, and ethical standards
Required Qualifications
  • Compliance with all state board requirements, including fingerprinting and application submission
  • Proficiency with telehealth platforms and remote clinical workflows
  • Experience in a mental health clinic or medical office with an understanding of patient workflows
  • Experience in a fast-paced call center environment requiring immediate follow-up
  • Ability to work independently and collaboratively within an interdisciplinary team
