Job Summary
A company is looking for a Newborn/Pediatric Nurse Care Manager.
Key Responsibilities:
- Deliver coordinated, patient-centered virtual Care Management by telephone and video to improve health outcomes
- Create impactful care plans with members and the care team to achieve desired goals
- Assist members in navigating complex medical conditions, treatment pathways, and the healthcare system
Required Qualifications:
- Bachelor of Science in Nursing (BSN)
- Active Compact RN license in good standing and California Nursing License
- 5+ years of Nursing experience, with 2+ years in a Newborn/Pediatric setting
- Current CCM Certification or eligibility to sit for the exam in August 2025
- Willingness to work Monday-Friday, 9am-6pm PST
