Job Summary

A company is looking for a remote Pharmacist.

Key Responsibilities

• Provide telephonic counseling, education, and support to patients regarding medication therapy and regimens
• Interface with patient providers and pharmacies to resolve medication-related issues
• Work collaboratively with the Pharmacy Advisor operations team to enhance patient health outcomes

Required Qualifications

• 1+ years of experience in Retail, LTC, Hospital, or PBM pharmacy, or pharmacist-to-patient consultation
• Bachelor's degree in pharmacy or Pharm D
• Active pharmacist license in AZ, HI, TX, IL, FL, or PA
• Preferred experience with patients having chronic disease states, especially diabetes
• PC literate with knowledge of pharmacy laws and drug utilization review
