Job Summary

A company is looking for a CA Licensed Physical Therapist (Care Navigator) - REMOTE.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct users in therapeutic exercise, neuromuscular re-education, and functional training through the technology platform
• Maintain records and notes while providing remote monitoring and assist in improving platform processes

Required Qualifications

• Active PT license in CA
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with access to a newer model laptop and smartphone
• Experience in behavioral modification or Health Coach Certification is a plus
