Job Summary

A company is looking for a California Licensed Physical Therapist (Care Navigator) to support patient success through remote monitoring and interventions.

Key Responsibilities

• Provide remote monitoring services for patients in home-exercise therapy programs
• Direct and assist users in therapeutic exercises and functional training via a technology platform
• Maintain records and provide feedback on patient progress to clinicians

Required Qualifications

• Active California PT license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy, comfortable with multi-platform environments
• Experience in behavioral modification; Health Coach Certification is a plus
