Job Summary

A company is looking for a Physician Assistant (PA-C) or Nurse Practitioner (NP).

Key Responsibilities

• Conduct in-depth medical, psychosocial, and risk evaluations
• Design and implement customized care plans in collaboration with the care team
• Provide patient education on disease management and lifestyle changes

Required Qualifications

• Active, unrestricted California PA-C or NP license
• National certification (NCCPA for PAs; AANP or ANCC for NPs)
• At least 2 years of direct clinical experience in relevant fields
• Experience with chronic care management or remote patient monitoring preferred
• Skilled in EHR documentation and familiar with telehealth platforms
