Job Summary

To support members in achieving diabetic reversal and overall health improvement, a California Licensed Physician Assistant will provide remote care management, conduct health assessments, and collaborate with a team of healthcare professionals, working part-time with specific hours from Tuesday to Friday.

Key responsibilities

• Engage with a collaborative team of healthcare professionals to provide comprehensive care
• Conduct health assessments and manage treatment plans for high-risk populations
• Educate patients on health habits and document care in the clinical platform

Required qualifications

• California Physician Assistant license; additional state licenses preferred
• 3-5 years of experience as an advanced practice provider
• Experience managing patients remotely across different geographic areas
• Board certification in family or internal medicine
• Proven experience working with underserved populations
