California Licensed Population Outcomes Manager

Job Summary

A company is looking for a Population Outcomes Manager - West Coast.

Key Responsibilities

• Analyze utilization trends and collaborate with interdisciplinary teams to develop interventions
• Screen market populations to identify patients at risk for increased utilization and implement strategies to reduce hospitalizations
• Design and manage projects in partnership with clinical leaders, ensuring effective tracking and reporting of outcomes

Required Qualifications

• Current California RN License or multistate RN license with willingness to obtain California license
• Bachelor's degree or equivalent experience; Master's degree preferred
• Four or more years of clinical experience, including two years in a value-based care environment
• Experience in geriatric care and population health is highly desired
• Proficiency in report generation and data analysis, particularly in Excel
