California Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (CA license required).

Key Responsibilities

• Support delivery of health care to patients by performing prescribed activities under the direction of Nurse Practitioners
• Provide one-on-one coaching to patients managing chronic conditions and help them execute personalized care plans
• Conduct comprehensive assessments and monitor patients' progress through regular check-ins and remote tools

Required Qualifications, Training, and Education

• Active multi-state compact Practical Nurse License
• Active California State LPN/LVN license is required
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for individuals managing chronic conditions
• Prior experience supporting patients remotely is a plus
